Pain in upper and lower right quadrants?

My little brother started complaining of cramping and pain in these quardrants last night,he has taken Pepto for it,but it isn't getting better.I'm afraid appendicitis is a possibility,because I had that last year and the doctors kept overlooking it,saying it wasn't possible.Three weeks later,I had to have an emergency appendectomy.Apparently,I didn't have the major symptoms,i.e. vomiting,fever,and crazy blood count. So,should I be worried about him? Is there anything to watch for?Thanks!
